Sensitive Health Information

Certain information, including mental health information and HIPAA-defined psychotherapy notes, may receive additional confidentiality protections. Most uses and disclosures of psychotherapy notes require written authorization unless otherwise permitted or required by law. If we receive or maintain substance use disorder records protected by 42 CFR Part 2, those records will be handled in accordance with applicable federal law.

Protecting Your Information

We are required to maintain the privacy and security of your protected health information. If a breach occurs that may have compromised the privacy or security of your information, we will notify affected individuals as required by law. We will not use or disclose protected health information except as permitted or required by law or as authorized by you.
